Community Health Workers (CHWs) are trusted members of your community who are here to help! They connect individuals and families to health care, food, housing, and other essential services. CHWs also provide support, guidance, and advocacy to improve overall well-being.

You may qualify for free Community Health Worker services! Let a CHW help you get the support you need. 

What’s Included in your CHW Program? 

• Finding Doctors and getting and appointment for physical, social, or mental needs.
• Keeping all your Provides fully informed of your health.
• Setting up transportation to your Doctor visits. 
• Connecting you to local resources such as food or other social services. 

How CHW Works

1. Initial Meeting – We’ll start with a friendly meeting to understand your needs and create a tailored care plan as the foundation for your health journey.
2. Creating Your Own Care Plan – Together, we’ll create a personalized care plan with clear steps and resources to help you take control of your health.
3. Regular Check-ins – Our team will check in regularly to adjust your care plan as needed, ensuring it stays effective and responsive.
